This 10-question survey is being distributed to help gather feedback on how to maximize practice attendance and practice enjoyment. The club officers will use the feedback for decision making.
The survey is anonymous, you are only asked to provide your city and zip code. The survey will close Friday, October 24, but please take it now. It should take 5 minutes or less.

Even if you are not a polo regular, you feedback is still helpful, however I recommend only taking this survey if you currently live in NE Ohio and maintain water polo interest.

- The full results will be used by officers and some info may be shared, but none that would allow identification of respondents
